what is the respective beneficiary number. Information is also requested from all dependents of the same and who
benefit or have benefited from this health subsystem. If so, it is requested that all contributions from the
beneficiary and their dependents and the reason for them. If there is a possibility of identifying the medicines prescribed to these beneficiaries and identifying places where medicines and/or
health care provided. It is further requested that, as soon as available, any of the requirements be the same
