Information is also requested from all dependents of the same and who<br>benefit or have benefited from this health subsystem. If so, it is requested that all contributions from the<br>beneficiary and their dependents and the reason for them. If there is a possibility of identifying the medicines prescribed to these beneficiaries and identifying places where medicines and\/or<br>health care provided.
